- Assessment of functioning in older adults hospitalized in long-term care in Portugal: analysis of a big dataPublication . Ramos, Ana; Fonseca, César; Pinho, Lara; Lopes, Manuel; Brites, Rui; Henriques, AdrianaBackground: Functioning assessment is a key tool for health professionals to characterize the person's degree of dependence and plan care. Objectives: The objectives were: (1) know the functioning components of older adults hospitalized in the National Network of Continuous Integrated Health Care (NNCIHC) in Portugal; and (2) compare the conceptual frameworks used in this network with the International Classification of Functioning, Disability and Health (ICF). Methods: A longitudinal retrospective study is made with 171,414 individuals aged 65 years and over. The Principal Components Analysis (PCA) was realized to reduce the number of variables, previously suggested by a scoping review, about the concepts that characterize the functionality. Then, a consensus meeting was held, where the items were matched with the ICF. Results: The average age of the sample is 80.17 years old (SD = 7.383), predominantly female (59%), without a spouse (54%), and with <6 years of education (56.4%). Four concepts were grouped: mobility, life daily activities, instrumental activities, and cognitive status that demonstrated good internal consistency. Most items correspond to ICF, except for the item "taking medication." Conclusion: Theoretical and conceptual similarities support the use of instruments based on the ICF in Portugal's healthcare network. We suggest that ICF also encompasses a specific dimension related to medication management, given its importance for people's health.
- Biopsychosocial factors that influence the purpose in life among working adults and retireesPublication . This cross-sectional study includes a sample of 1330 participants, of whom 62.2% were female, with ages ranging from 55 and 84 years, with a mean of 61.93 years and a standard deviation of 7.65. Results suggest that the education level, stress, spirituality (religion) and optimism, social support from friends, and quality of life related to physical health seem to contribute positively to the PIL for both groups. However, some variables such as age, marital status and environmental quality of life help explain the PIL of retired people and the quality of life related to social support helps explain the PIL of working adults. Overall, the reported findings suggest that the purpose in life is strongly related to physical, psychological, social and environmental health factors. It is highlighted that working adults and retired people have their purpose in life related to similar factors and others specific to each life stage, suggesting the need for crucial interventions to promote a healthier and more positive aging process.
- Construção e validação da tabela nacional de funcionalidade para as doenças crónicasPublication . Pereira, Carla Sandra; Banco, Jaime; Lopes, Manuel; Escoval, Ana; Nogueira, Paulo; Diniz, José Alexandre; Guerra, Fernando; Coelho, AnabelaIntrodução: A avaliação sistemática e registo da funcionalidade de pessoas adultas com doença crónica permite horizontalizar políticas de saúde, sociais e emprego de acordo com a funcionalidade; dotar os profissionais de saúde e sociais de um instrumento de recolha de informação, que complemente os registos de doença; medindo os ganhos de funcionalidade. O objetivo de estudo foi desenvolver uma Tabela Nacional de Funcionalidade para adultos em idade ativa com doença crónica, de acordo com a Classificação Internacional de Funcionalidade, Incapacidade e Saúde da Organização Mundial de Saúde. Material e Métodos: Recorremos a métodos quantitativos e qualitativos; revisão da literatura (17 artigos), grupo focal (nove peritos), painel de Delphi (16 peritos) e estudo exploratório (309 pessoas com doença crónica). Resultados: Na revisão da literatura, foram identificadas 67 atividades limitadas na população em estudo das quais foram selecionadas 40 atividades pelo grupo focal e 38 validadas pelo painel de Delphi. Discussão: Para testar as propriedades psicométricas comparamos o valor médio de todos os coeficientes possíveis do tipo consistência interna (split-half). Na análise da discriminação dos níveis de funcionalidade em amostras diferentes, verificou-se a igualdade de variâncias pelo teste de Levene e a igualdade de média por recurso ao teste t. De acordo com a observação e análise do coeficiente α de Cronbach, verificou-se que a Tabela Nacional de Funcionalidade proposta apresenta bons níveis de fiabilidade. Na análise de componentes principais, identificaram-se cinco dimensões. Conclusão: A referida tabela tem características psicométricas apropriadas no que diz respeito à consistência, fiabilidade e validade interna.
- Functional profile of older adults hospitalized in convalescence units of the national network of integrated continuous care of Portugal: a longitudinal studyPublication . Ramos, Ana; Fonseca, César; Pinho, Lara; Lopes, Manuel; Oliveira, Henrique; Henriques, AdrianaAim: To evaluate the evolution of the functional profile of older adults admitted to a health unit in Portugal; to relate the functional profile of these individuals with age, sex, education level and emotional state; and to evaluate the probability of the degree of dependence as a function of age and sex. Methods: longitudinal, retrospective study with a sample of 59,013 older adults admitted to convalescence units of the National Network of Integrated Continuous Care of Portugal. Results: In the first 75 days of hospitalization, activities of daily living, mobility and cognitive state improved, but there was a decline after 75 days of hospitalization. The ability to perform instrumental activities of daily living improved in the first 15 days of hospitalization, stabilized until 45 days and then began to worsen. Women had a higher probability of having a severe/complete dependence three years earlier than men (88 years to 91 years). A higher education level and stable emotional state were protective factors against functional decline. Conclusions: The functional profile of older adults improved during the length of stay recommended for hospitalization in convalescence units (30 days). It is critical for health systems to adopt strategies to prevent declines in the emotional state of frail individuals.
