A Colangite Biliar Primária (CBP) é uma doença autoimune hepática colestática crónica. Caracterizada por destruição das vias biliares e colestase intrahepática progressiva. Afeta maioritariamente mulheres na 5ª ou 6ª década de vida. A terapêutica de 1ª linha é o Ácido Ursodesoxicólico (AU), que é também a terapêutica de manutenção [1]. Reportamos um caso de uma doente do sexo feminino de 47 anos, com CBP refratária ao tratamento com AU. Realizou switch terapêutico para Ácido Obeticólico. Com este Caso Clínico pretende-se descrever o controlo de doença e sintomático da doente, baseado na colheita de dados do processo físico e digital. Para contextualizar, será feita uma revisão sistemática da terapêutica atual na CBP.
Primary biliary cholangitis (PBC) is an autoimmune chronic cholestatic liver disease characterized by biliary destruction and progressive intrahepatic cholestasis. PBC primarily affects women in their fifth or sixth decade of life. First-line therapy to slow disease progression is ursodeoxycholic acid (UDCA), which is the mainstay of pharmacologic therapy for all patients with PBC [1]. Herein, we report a case of a 47 yo woman with PBC whose disease is refractory to treatment with UDCA. Therapy has been switched to Obeticholic Acid. This case report describes disease and symptom management, based on research on both physical and online documents regarding the patient. In order to contextualize it, we review current therapy in Primary Biliary Cholangitis based on online research.
