Hypertension, a risk factor for several cardiovascular diseases, is one of the most common diseases in the world, affecting 1 billion people and estimated to cause nine million deaths/year. Proper lifestyle changes and medication are the most effective therapeutic approaches on the different stages of hypertension. The belief in "what is natural is good" can lead to the use of plants and supplements for certain diseases management, including hypertension. Only thirteen of the antihypertensive medicinal plants described in this monograph have their use in hypertension treatment supported by strong evidence of efficacy and safety, namely: Allium sativum, Apium graveolens, Avena sativa, Cammelia sinesis, Crataegus spp., Eucommia ulmoides, Hibiscus sabdariffa, Glycine max, Lycoperisicon esculentum, Linum usitatissum, Olea europaea, Pinus pinaster, Sesamum indicum. The more effective and safer plants for HT management are Olea europaea and Hibiscus sabdariffa, while Rauwolfia serpentine use raises safety concerns including drug-interactions. Allium sativum has the highest number of potential drug-interactions. In contrast, all supplements addressed are supported by randomized clinical trials. CoQ10 is the one that shows best safety and efficacy results on HT management. Potential plant/supplement-drug interactions have also been addressed but further clinical studies are needed to understand the clinical significance of those interactions.
