A Doença de Crioaglutininas representa 15 a 20% dos casos de anemia hemolítica imunomediada. Em cerca de 90% dos casos, este diagnóstico é secundário a outra patologia, prevalecendo a etiologia infeciosa ou autoimune nos doentes mais jovens e as doenças hemato-oncológicas nas faixas etárias superiores. Ainda assim, admite-se atualmente que a forma primária integra o especto de distúrbios linfoproliferativos da medula óssea e não é um simples fenómeno isolado e idiopático, associando-se potencialmente a doença hemato-oncológica em evolução. Apresenta-se um caso raro de Doença de Crioaglutininas como manifestação grave e inaugural de linfoma de células B subclínico.
Cold Agglutinin Disease represents 15 to 20% of immune-mediated hemolytic anemia cases. In about 90% of cases, this diagnosis is secondary to another pathology, with an infectious or autoimmune etiology prevailing in younger patients and hematooncological diseases in older age groups. Even so, it is currently accepted that the primary form integrates the spectrum of lymphoproliferative disorders of the bone marrow and it is not a simple isolated and idiopathic phenomenon, potentially associated itself with an evolving hemato-oncological disease. The author presents a rare case of Cold Agglutinin Disease as a severe and inaugural manifestation of subclinical B-cell lymphoma.
