Índice de regressão dentária numa população dos 6 aos 15 anos: aplicação na estimativa médico-legal da idade

Introdução: A estimativa de idade na Medicina Dentária Forense é importante para aplicação da norma em várias áreas do direito com interseção com a Medicina Legal e Ciências Forenses. A fórmula Europeia de Cameriere é um dos métodos usados para estimar a idade, em que utiliza a distância entre os ápices abertos e se divide pelo comprimento do dente, obtendo, desta forma, o índice de maturidade de sete dentes.
Objetivo: O objetivo deste estudo foi estimar a idade cronológica no âmbito da identificação médico-legal da idade, recorrendo ao índice de regressão dentária europeu, bem como, os respetivos estádios de Demirjian, numa população portuguesa, dos 6 aos 15 anos.
Materiais e Métodos: Selecionaram-se 483 ortopantomografias nas quais se realizaram as medições necessárias, recorrendo ao método de Cameriere, no programa ImageJ. Todos os sete dentes permanentes do 3.º Quadrante foram classificados de acordo com os estádios de Demirjian, de A-H.
Resultados: Os coeficientes de correlação interclasse apresentaram valores, na sua quase totalidade, superiores a 0,75. O coeficiente de determinação obtido através da fórmula europeia é igual a 80,9%. A classificação pelos estádios de Demirjian mostrou uma sobrestimação média de 8,7 meses. Não foram detetadas diferenças estatisticamente significativas na precisão da estimativa da idade entre os dois sexos.
Conclusão: Não existem diferenças estatisticamente significativas entre a idade cronológica e a idade dentária dos indivíduos, estimada pelo índice de regressão dentária europeu, quando utilizados coeficientes estimados para a população Portuguesa, mas verificaram-se diferenças entre a idade cronológica e a idade dentária estimada pelos estádios de Demirjian. Não existem diferenças estatisticamente significativas entre géneros na aplicação dos dois métodos.
Introduction: Age estimation in Forensic Dental Medicine is important for the application of the standard in several areas of law intersecting with Legal Medicine and Forensic Sciences. The European Cameriere formula is one of the methods used to estimate age, in which it uses the distance between the open apices and is divided by the length of the tooth, thus obtaining the maturity index of seven teeth. Objective: The objective of this study was to estimate chronological age in the context of medico-legal age identification, using the European dental regression index, as well as the respective Demirjian stages, in a Portuguese population, from 6 to 15 years old. Materials and Methods: 483 orthopantomographs were selected in which the necessary measurements were made, using the Cameriere method, in the ImageJ program. All seven permanent teeth in the 3rd Quadrant were classified according to the A-H Demirjian stages. Results: The interclass correlation coefficients showed values, almost entirely, greater than 0.75. The coefficient of determination obtained using the European formula is equal to 80.9%. Classification by Demirjian stages showed an average overestimation of 8.7 months. No statistically significant differences were found in the accuracy of the age estimate between the two sexes. Conclusion: There are no statistically significant differences between the chronological age and the dental age of the individuals, estimated by the European dental regression index, provided it is used estimated coefficients for the Portuguese population. Moreover, statistically significant differences were found between the chronological age and the dental age by the Demirjian stages. There are no statistically significant differences between genders in the application of these two methods.
