A piometra é uma doença do foro reprodutivo com origem multifatorial, resultando da interação entre a infeção bacteriana, o estímulo hormonal, a predisposição genética e as alterações uterinas pré-existentes, que afeta sobretudo cadelas seniores e geriátricas. Existem dois tipos de abordagem para esta doença, a cirúrgica, que implica a remoção dos ovários e útero, e a médica, cujo objetivo é neutralizar ou eliminar o estímulo hormonal. Através de um estudo retrospetivo, descritivo e observacional foram avaliados doze casos de piometra, tendo em consideração aspetos clínicos, laboratoriais e imagiológicos. A taxa de sucesso terapêutico foi de 81,8% num primeiro ciclo de tratamento, com recidiva observada em 30% dos casos. A escolha pela abordagem médica esteve frequentemente associada à existência de comorbilidades, idade avançada ou interesse reprodutivo. Os resultados sugerem que, em casos selecionados, a aglepristona pode representar uma alternativa viável à ovariohisterectomia. Esta abordagem requer um seguimento clínico rigoroso, pois são necessárias várias administrações até à resolução completa da doença. A ovariohisterectomia continua a ser o tratamento de eleição, por permitir a resolução definitiva da doença, com uma reduzida probabilidade de recidiva. Independentemente da abordagem escolhida, deve ser sempre realizada antibioterapia, garantindo o controlo da infeção e a eficácia do tratamento
Pyometra is a multifactorial reproductive disease resulting from the interaction between bacterial infection, hormonal stimulation, genetic predisposition, and pre-existing uterine alterations. It predominantly affects senior and geriatric female dogs. There are two different treatment options: surgical intervention, involving the removal of the ovaries and uterus, and medical therapy using pharmacological agents aimed at neutralizing or eliminating hormonal stimulation. In a retrospective, descriptive, and observational study, twelve cases of pyometra were evaluated, taking into account clinical, laboratory, and imaging findings. The therapeutic success rate was 81.8% after the first treatment cycle, with recurrence observed in 25% of cases. The choice of medical management was often associated with the presence of comorbidities, advanced age, or reproductive interest. The results suggest that, in selected cases, aglepristone may represent a viable alternative to ovariohysterectomy, with its use being more justified in breeding females. This approach requires close clinical monitoring, as multiple administrations are needed until complete resolution of the disease. Ovariohysterectomy remains the treatment of choice, as it provides definitive resolution of the condition with a low probability of recurrence. Regardless of the chosen approach, appropriate antibiotic therapy should always be administered to ensure infection control and treatment efficacy
