Contexto: A terapia de ressincronização cardíaca (TRC) é recomendada para reduzir a mortalidade e morbilidade em doentes com insuficiência cardíaca crónica (ICC), cujos sintomas permanecem na classe II-IV da New York Heart Association (NYHA).
Objetivo: O objetivo do estudo foi caracterizar a composição corporal e a aptidão física funcional destes doentes com TRC e verificar associações.
Métodos: O estudo foi transversal observacional e descritivo, com uma amostra de 18 doentes. A densitometria radiológica de dupla energia (DEXA) foi utilizada para estimar a composição corporal. A prova de esforço cardiopulmonar e o teste de caminhada de seis minutos (6MWT) foram realizados para caracterizar a capacidade cardiorrespiratória. O teste de levantar e sentar na cadeira avaliou a força dos membros inferiores, e o teste do dinamómetro a força dos membros superiores.
Resultados: A amostra apresentou um índice de massa muscular esquelética (MME) com um risco moderado de incapacidade física. O teste do dinamómetro apresentou associação positiva forte com os níveis de MME. No teste dos 6MWT a amostra apresentou resultados <450 m de distância percorrida, apresentando um mau prognostico clínico.
Conclusão: Nestes doentes a MME encontra-se diminuída, apresentando uma reduzida capacidade de produção de força e limitações na qualidade da marcha. Concluímos que apresentam limitações na sua capacidade física funcional.
ABSTRACT Background: Cardiac resynchronization therapy (CRT) is recommended to reduce mortality and morbidity in patients with chronic heart failure (CHF) whose symptoms remain in class II-IV New York Heart Association (NYHA). Objective: The main goal of the current study was to characterize the body composition and functional physical fitness of patients with CRT, and verify the association. Methods: The study was observational and descriptive dealing with 18 patients. Dual energy X-ray absorptiometry was used to estimate body composition. Cardiopulmonary exercise testing (CPET) was conducted as well as the six minute walk test (6MWT) to characterize cardiorespiratory capacity, the 30 seconds chair stand test was used to assess lower limb strength while the handgrip test was conducted to estimate upper limbs strength. Results: The sample had a skeletal muscle mass index (SMM) with a moderate risk of physical disability. The test dynamometer showed a strong correlation with the levels of SMM. In the test of the sample had 6MWT results <450m distance representing a poor clinical prognosis. Conclusion: In these patients, the SMM is decreased. The results showed a reduced ability to produce force and limitations in walking capacity therefore we conclude that this population presents limitation in their functional fitness.
