Introdução: A fraca qualidade óssea associada à tendência progressiva de reabsorção após perda dentária, com consequente pneumatização do seio maxilar, fazem com que reabilitar a maxila posterior constitua, desde sempre, um desafio. De forma a solucionar esta questão, diferentes protocolos de tratamento têm sido propostos. Objetivo: Realizar uma análise comparativa entre a elevação de seio maxilar e a ancoragem zigomática, na reabilitação da maxila atrófica. Materiais e métodos: No âmbito desta revisão da literatura, foi efetuada uma pesquisa de artigos científicos, com base na pergunta PICOS, recorrendo às bases de dados Pubmed e Cochrane. Resultados: Ambas as técnicas cirúrgicas abordadas são estratégias de tratamento viáveis na reabilitação implanto-suportada da maxila atrófica. A elevação de seio maxilar, por estar amplamente estudada e ser considerada uma prática previsível pela literatura, é uma técnica de eleição neste tipo de reabilitação. A ancoragem zigomática é um procedimento promissor que, ao apresentar complexidade de execução, e ao carecer de evidência científica que avalie a sua previsibilidade a longo prazo, vê a sua inclusão na prática diária dificultada. Conclusão: A opção pela técnica a utilizar deve envolver uma avaliação criteriosa do paciente, de forma a garantir a sobrevivência e o sucesso do tratamento a longo prazo. Cabe ao clínico, em conformidade com o paciente, escolher a técnica mais adequada a cada situação.
Background: A poor bone quality associated with a progressive tendency of resorption after tooth loss, with consequent pneumatization of the maxillary sinus, make the rehabilitation of posterior maxilla a challenge. In order to solve this question, different treatment protocols have been proposed. Purpose: Perform a comparative analysis between sinus lift and zygomatic anchorage, in rehabilitation of the atrophic maxilla. Materials and Methods: A literature search, based on the question PICOS and relying on Pubmed and Cochrane databases, was performed, in order to retrieve relevant articles. Results: Both surgical techniques are sustainable procedures in implant-supported rehabilitation of the atrophic maxilla. As it is widely studied and considered a predictable practice by the literature, sinus lift procedure is an excellence technique in this type of rehabilitation. The zygomatic anchorage is a promising procedure. However, due to the complexity of the technique, and the lack of scientific evidence to assess its long-term predictability, the inclusion in daily practice has been difficulted. Conclusion: The choice of technique used should involve a careful evaluation of the patient, to ensure the survival and successful long-term rehabilitation. It is up to the dentist, in accordance with the patient, to choose the most appropriate technique for each situation.
