Purpose: To evaluate the postoperative pain after using a bioceramic material (BioRoot™ RCS) as an endodontic sealer with single cone technique, and compare the results with a resin endodontic sealer (AH Plus) with the cold lateral condensation technique, and using the continuous wave of condensation. Materials and Methods: 30 patients were divided randomly in 10 patients for each group of obturation system: BioRoot™ RCS + single cone technique, AH Plus + cold lateral condensation and AH Plus + continuous wave of condensation. Teeth were cleaned, shaped, and obturated during the patients’ first visit or only obturated on the second visit, when possible. BioRoot™ RCS (Septodont) using single cone technique, was applied in a randomnized selected group. The rest of the sample was obturated with AH Plus, applying the cold lateral condensation or applying the continuous have of condensation (B&L Biotech). At the end of the visit, patients were given a VAS form, on which they were requested to rate their pain during the 7 days post-treatment. Results: Single cone + BioRoot™ referred post-operative pain more frequently (50 %, n = 5) than Continuous wave + AH Plus (30 %, n = 4) or Lateral condensation + AH Plus (20 %, n = 2). Single cone + BioRoot™ also presented the highest percentage of moderate post-operative pain intensity felt during the 7 day evaluation period (20 %, n = 2 vs. 10 %, n = 1 for Lateral condensation and 0 % for Continuous wave). Lateral condensation + AH Plus presented the least post-operative pain frequency, but Continuous wave + AH Plus revealed the least levels of pain regarding the highest pain intensity felt during the 7 day evaluation period. Conclusions: Comparisons between the three obturation systems did not reveal any statistically significant differences regarding the highest pain intensity felt during the 7 day evaluation period or according to pain intensity between days. More studies with a larger sample are needed to obtain more conclusive results.
