Mesotelioma peritoneal multiquístico benigno (MPMB) é uma lesão rara que ocorre predominantemente no sexo feminino em idade fértil ou na meia-idade. A sua associação com o cistadenoma mucinoso é de extrema raridade. Relatamos o caso duma mulher de 63 anos, assintomática, com antecedentes de histerectomia total com anexectomia bilateral por patologia uterina benigna, em que foi detetada uma volumosa massa abdomino-pélvica, numa TAC realizada para esclarecimento de marcadores tumorais aumentados. Foi submetida a laparotomia exploradora com remoção da massa tumoral, tendo-se detetado a presença de múltiplas formações quísticas com dimensões variando entre 2 mm e os 3 cm de diâmetro, e que envolviam o peritoneu, o apêndice ileo-cecal e o epíploon. O estudo imunohistoquimico da massa (30 cm diâmetro e 3.304 g de peso) revelou um Cistadenoma Mucinoso (interpretado como extra-ovárico devido aos antecedentes cirúrgicos da doente) enquanto que a imunohistoquímica dos restantes quistos envolvendo as outras estruturas removidas, revelou um Mesotelioma Peritoneal Multiquístico Benigno. A doente mantém seguimento regular (nos últimos 3 anos) em Consulta de Ginecologia, apresentando bom estado geral e sem evidência clínica ou imagiológicos de recidiva da doença. De acordo com a literatura consultada, só encontrámos um caso com a associação destas duas patologias.
Benign multicystic peritoneal mesothelioma (BMPM) is a rare lesion that occurs predominantly in females of childbearing age or middle age. Its association with mucinous cystadenoma is extremely rare. We report the case of a 63-year-old woman, asymptomatic, with a history of total hysterectomy with bilateral adnexectomy due to benign uterine pathology, in which a large abdominal-pelvic mass was detected in a CT scan performed to clarify increased tumor markers. She was submitted to exploratory laparotomy with removal of the tumor mass, and with the finding of multiple cystic formations with dimensions varying between 2 mm and 3 cm in diameter, involving the peritoneum, ileo-cecal appendix and epiploon. The immunohystochemical study of the mass (30 cm in diameter and 3,304 g in weight) revealed a Mucinous Cystadenoma (interpreted as extra-ovarian due to the patient's surgical history), while the immunohystochemistry of the remaining cysts involving the other removed structures revealed a Benign Multicystic Peritoneal Mesothelioma. The patient maintains regular follow-up (for the past 3 years) in the Gynecology Outpatient Clinic, presenting good general condition and without clinical or imagological signs of relapse of the disease. According to the literature reviewed, we only found one case with the association of these two pathologies.
