Introdução e Objetivos: a influência das extrações de pré-molares durante o tratamento ortodôntico na posterior erupção dos terceiros molares tem sido objeto de debate nas últimas décadas. Assim, o objetivo deste estudo retrospetivo foi comparar o grau de erupção dos terceiros molares mandibulares entre pacientes tratados com e sem exodontia de pré-molares inferiores, utilizando radiografias panorâmicas pré e póstratamento. Materiais e Métodos: para este estudo selecionou-se uma amostra de conveniência composta por pacientes de um consultório privado com o tratamento ortodôntico finalizado, a qual foi eleita com base nos critérios de inclusão e exclusão previamente estabelecidos. No total, foram selecionados 18 pacientes (36 hemi arcadas), divididos posteriormente em dois grupos: Grupo S (pacientes tratados sem exodontias) e Grupo C (pacientes tratados com exodontias), tendo sido registada a Classe de Angle inicial. Analisaram-se as ortopantomografias destes pacientes pré e pós-tratamento (T1 e T2), através do registo de quatro medidas angulares e duas medidas lineares relativas à posição e angulação dos terceiros molares. O lado direito e esquerdo das radiografias foram avaliados de forma independente. A decisão sobre exodontia dos terceiros molares após tratamento tratamento ortodôntico foi igualmente registada. Foi executada a estatística descritiva das variáveis dependentes, assim como análises de variância (ANOVA). Resultados e Conclusões: a extração de pré-molares inferiores na terapêutica ortodôntica não altera significativamente a angulação dos terceiros molares em relação ao plano oclusal ou em relação aos segundos molares. No entanto, essa extração aumenta o espaço disponível para a erupção dos terceiros molares mandibulares, reduzindo significativamente a referência para a posterior remoção dos mesmos. Apesar das suas limitações, este estudo poderá ter relevância clínica em casos borderline, nos quais a opção de extrair pré-molares pode ser preferível quando existe uma grande probabilidade de os terceiros molares virem a erupcionar corretamente no futuro.

Therefore, the aim of this retrospective study was to compare the degree of eruption of mandibular third molars between patients treated with and without extraction of inferior premolars, using panoramic radiographs pre and posttreatment. Materials and methods: we selected a convenience sample of patients from a private dental clinic, who had finished their orthodontic treatment, based on the inclusion and exclusion criteria previously established. In total, 18 patients were selected (36 third molars), which were then divided into two groups: Group S (patients without extractions) and Group C (patients treated with extraction). The initial Angle classes were recorded. After that, we studied the panoramic radiographs before (T1) and after (T2) orthodontic treatment, through four angular measurements and two linear measurements regarding the third molar position. The right and left sides were evaluated independently. The decision of extraction of the third molars after orthodontic treatment was also recorded. For each group and each time, we performed a descriptive statistic of the dependent variables and also the analysis of variance (ANOVA). Results and Conclusions: the extraction of inferior premolars in orthodontic treatment does not significantly alter the angulation of the third molars in relation to the occlusal plane or in relation to the second molars. However, such extraction increases the space available for the eruption of the mandibular third molars, significantly reducing the reference for the subsequent removal of those teeth. Despite its limitations, this study may have clinical relevance in borderline cases, in which the option of extracting premolars may be preferable when there is a high probability that the third molars come to erupt properly in the future.
