O número de profissionais de saúde do Serviço Nacional de Saúde (SNS) tem sido significativamente reforçado ao longo dos últimos anos. Apesar disso, o aumento da produção do SNS não foi proporcional ao dos recursos humanos. De facto, o SNS continua a enfrentar grandes desafios, nomeadamente ao nível do acesso a cuidados de saúde – tal como demonstrado pelos elevados tempos de espera ou pelo número de utentes sem médico de família. O objetivo deste estudo é analisar a eficiência dos hospitais do SNS, estudando a sua relação com os recursos humanos do sistema de saúde. Foram estimadas funções de produção estocásticas para um conjunto de 39 hospitais públicos com dados mensais entre 2015 e 2023. Consideram-se como outputs o número de consultas, número de dias de internamentos e número de cirurgias. Estes outputs foram produzidos através dos seguintes inputs: número de médicos, número de enfermeiros e staff (assistentes operacionais e técnicos superiores de saúde). Os resultados indicam um nível médio de ineficiência global nos hospitais públicos portugueses em cerca de 14%, mas é nos internamentos que o SNS é mais eficiente, com 95% de eficiência média global. No entanto, não se verifica uma relação forte entre a eficiência e os recursos humanos. Enquanto existe uma fraca tendência negativa entre a eficiência e o rácio de enfermeiros por médico no modelo com o output consultas, existe uma fraca tendência positiva nos restantes outputs.
The number of health professionals in the National Health Service (SNS) has been significantly increased in recent years. Despite this, the increase in SNS output has not been proportional to the increase in human resources. In fact, the SNS continues to face major challenges, particularly in terms of access to healthcare - as demonstrated by the high waiting times or the number of users without a family doctor. The aim of this study is to analyse the efficiency of SNS hospitals by studying its relationship with the health system's human resources. Stochastic production functions were estimated for a set of 39 public hospitals with monthly data between 2015 and 2023. The outputs were the number of appointments, number of hospitalisation days and number of surgeries. These outputs were produced using the following inputs: number of doctors, number of nurses and staff (operational assistants and superior health technicians). The results indicate an average level of overall inefficiency in Portuguese public hospitals of around 14 per cent, but it is in admissions that the SNS is most efficient, with 95 per cent overall average efficiency. However, there is no evidence of a strong relationship between efficiency and human resources. While there is a weak negative trend between efficiency and the ratio of nurses to doctors in the model with the output consultations, there is a weak positive trend in the other outputs.
