There is a growing number of review papers on amyotrophic lateral sclerosis (ALS) in the literature which mirrors recent scientific developments and the importance of these articles to the readers and journals. The article published in this journal and written by two very respected and productive authors focuses on biomarkers of disease progression in ALS. I certainly agree with the “3Rs rule (replicate, replicate, replicate)”, but other important features of a good biomarker to quantify disease progression are missing, including: sensitivity (otherwise most trials will fail to show minor but significant drug effects); tolerability and speed of performance (to avoid large patient dropout); technological requirements and costs (unless aimed for application in a few centers only); and correlation with prognostic clinical markers (clinical significance is meaningful).
