A perturbação depressiva e a insuficiência cardíaca são duas condições patológicas distintas, porém com alguns aspetos fisiopatológicos comuns. A insuficiência cardíaca é um síndrome complexo na qual o funcionamento correto do coração encontra-se comprometido. A insuficiência cardíaca tem-se tornado um problema major de Saúde Pública e, com o envelhecimento populacional assim como com o aumento da sobrevivência após eventos cardiovasculares, é inevitável que continue a ser um desafio importante. Neste contexto, é importante identificar possíveis fatores de risco modificáveis de modo a oferecer intervenções preventivas e terapêuticas eficazes. Fatores como alterações ao nível do eixo hipotálamo-hipofisário, do endotélio, do sistema nervoso autónomo e vias de inflamação são apontados como possíveis mecanismos de relação entre as duas entidades abordadas. A insuficiência cardíaca e a depressão constituem problemas de saúde com uma relação bidirecional, pelo que, uma intervenção terapêutica eficaz terá que ser realizada por uma equipa multidisciplinar composta por cardiologistas, psiquiatras, psicólogos e médicos de família.
O presente trabalho visa rever a relação entre a depressão e a insuficiência cardíaca a nível de prevalência, mecanismo fisiopatológico, diagnóstico e terapêutica, através de uma revisão narrativa da literatura que inclui estudos publicados até Novembro de 2021.
Depression disorder and heart failure are two distinct pathological conditions, but with some common pathophysiological aspects. Heart failure is a complex syndrome in which the correct function of the heart is compromised. Heart failure has become a major public health problem and with the aging of the population as well as the increase in survival after cardiovascular events, it is inevitable that this will continue to be an important challenge. Therefore, it is important to identify possible modifiable risk factors for heart failure in order to offer effective preventive and therapeutic interventions. Alterations in the hypothalamic-pituitary axis, in endothelium, in autonomic nervous system and in the inflammation pathways are pointed out as possible related mechanisms between the two conditions addressed. Heart failure and depression are health problems with a bidirectional relationship, so an effective therapeutic intervention will have to be carried out by a multidisciplinary team composed of cardiologists, psychiatrists, psychologists and family doctors. The present work aims to review the relationship between depression and heart failure in terms of prevalence, pathophysiological mechanism, diagnosis and therapy, through a narrative review of literature that includes studies published until November 2021.
