A mielopatia por embolia fibrocartilaginosa (MEF) é uma mielopatia vascular que resulta da embolia de material do núcleo pulposo do disco intervertebral que origina uma interrupção do suprimento sanguíneo medular com consequente isquemia.
Foram incluídos neste estudo 16 cães cuja apresentação clínica se caracterizava por um início agudo e não progressivo, com ausência de hiperestesia espinhal e ressonância magnética ou tomografia computorizada compatível com MEF ou extrusão aguda não compressiva do núcleo pulposo. Foram avaliadas as variáveis raça, idade, género, etiologia, tipo de maneio prescrito, tipo de lesão, classificação neurológica quanto ao movimento observado, lateralização dos sinais clínicos, sensibilidade à dor profunda, sensibilidade à dor superficial, reflexos espinhais periféricos, reflexo cutâneo do tronco (RCT), período decorrido entre a lesão e o início do protocolo de neuroreabilitação funcional (NRF), duração do protocolo de NRF (treab), défices à saída do centro e recuperação. O objetivo deste estudo foi identificar se existia uma relação entre fatores observados à entrada do centro e o período de duração do protocolo NRF bem como com a recuperação do animal após a realização do protocolo.
A idade média apresentada foi de 5,3 anos e o rácio entre machos e fêmeas foi de 1,7:1. Foram representadas 12 raças diferentes, sendo a mais frequente o Labrador Retriever. Foi encontrada uma relação estatisticamente significativa entre as seguintes variáveis: porte e recuperação (p=0.046), lateralização e recuperação (p=0.018), reflexos espinhais periféricos e recuperação (p=0.007) e, por fim, RCT e o treab (p=0.004). Destes resultados, o RCT destaca-se por não ter sido mencionado noutros estudos sendo considerado um novo fator de prognóstico.
Concluindo, este estudo permitiu identificar alguns fatores indicadores de prognóstico tal como era desejado, tendo sido possível, inclusive, encontrar um fator de prognóstico que ainda não tinha sido referida para esta doença.
ABSTRACT - IDENTIFICATION OF PROGNOSTIC FACTORS IN FIBROCARTILAGINOUS EMBOLIC MYELOPATHY IN THE DOG - The fibrocartilaginous embolic myelopathy (FCEM) is a vascular myelopathy which results from embolism from the intervertebral disc nucleus pulposus material that causes an interruption of the spinal cord blood supply with consequent ischaemia. This study included 16 dogs with a history and clinical presentation and computerized tomography and magnetic resonance imaging results compatible with FCEM or acute noncompressive nucleous pulposus extrusion. The variables under study were breed, age, gender, aetiology, prescribed handling, lesion type, neurological classification regarding the observed movement, clinical signs lateralization, deep pain perception, superficial pain perception, spinal reflexes, cutaneous trunci reflex (CTR), time period between lesion and beginning of functional neurorehabilitation protocol (FNR), FNR protocol duration (treab), neurological deficits on leaving the centre and recovery. The aim of this study is to evaluate an association between variables taken in the admission point and the rehabilitation protocol duration and patient recovery after taking the protocol. A statistical significant association were found between breed size and recovery (p=0.046), lateralization and recovery (p=0.018), spinal reflexes and recovery (p=0.007) and CTR and treab (p=0.004). In conclusion, some prognostic factors were identified as desired, and it was possible to find a variable that had not yet been reported by others authors.
