dc.description.abstractIn the last decades, the frequency and nature of incidents reported in the field of radiotherapy have highlighted the need for customized, proactive risk management approaches, as opposed to traditional quality control programs. The application of risk analysis methodologies emerges as an adaptable alternative for mitigating potential failures that may occur throughout the radiotherapy process, allowing them to complement or replace fixed checklists, applied uniformly across all radiotherapy clinical units, without considering the complexity and organization of each unit. The main objective of this study is to identify the most critical potential risks in the radiotherapy department of SAMS and to propose appropriate mitigation measures (preventions and barriers), based on the Failure Modes and Effect Analysis (FMEA) methodology, in accordance with the recommendations proposed in the Task Group 100 (TG-100) report of the American Association of Physicists in Medicine (AAPM). The process map was developed through direct observation and by consulting the TG-100 and TG-275 reports, resulting in the identification of 183 failure modes. Each failure mode was assigned scores for severity, occurrence, and lack of detectability according to the scales provided in the TG-100 report, enabling the calculation of the Risk Priority Number (RPN) and subsequent analysis using the IBA MyQA Proactive software. The results demonstrated that the most critical step is volume delimitation, which is susceptible to human and organizational causes. The implementation of measures like checklists, peer review, continuous professional training, enhanced multidisciplinary communication demonstrated potential for reducing RPN values. It is therefore concluded that the implementation of tailored risk management mechanisms represents an essential tool to ensure the safety of procedures, patients, and healthcare professionals.en
