O Papiloma Invertido (PI) da região nasossinusal, apesar de ser uma entidade rara e benigna, é dotado de importante agressividade local e de potencial evolução maligna. Ao longo da história, mais de vinte e cinco termos descritivos diferentes foram utilizados para se referirem a esta lesão; no entanto, sabe-se atualmente que muitos destes se revelaram inapropriados. No que se refere à fisiopatologia, também a maioria dos mecanismos inerentes permanecem por esclarecer, não existindo uma teoria sólida que, por si só, explique fidedignamente o desenvolvimento fenotípico desta doença. Para a escassa investigação na área, contribui também o facto dos sinais e sintomas atribuíveis ao PI serem, inicialmente, subtis e inespecíficos e, aquando do desenvolvimento de um maior grau de lesão, se confundirem facilmente com aqueles atribuíveis a outras patologias.
Neste artigo de revisão, pretende-se abordar, de forma generalista, os mecanismos fisiopatológicos conhecidos do PI, bem como o diagnóstico e as estratégias terapêuticas mais vulgarmente utilizadas. Por último, serão discutidas as lacunas de conhecimento científico e clínico e como é que as mesmas poderão ser utilizadas para perspetivar a abordagem futura desta doença.
The Inverted Nasosinusal Papilloma (PI), although a rare and benign entity, is capable of important local aggressiveness and carcinogenic potential. Throughout the History, more than twenty five different descriptive terms have been used to describe this pathology; nowadays, it is quite clear that a lot of these are inappropriate. In what concerns the physiopathology, most of the underlying mechanisms remain unestablished and there is not a single solid theory that can, by itself, explain the phenotypical development of this disease. Furthermore, the scant research in this field also reflects the fact that the signs and symptoms at the early stages of the disease are often subtle and unspecific; in later development, they are easily assumed as being compatible with other diseases. This article aims to review the literature on PI mainly on its physiopathological mechanisms, as well as on its diagnosis and most commonly used therapeutic strategies. Finally, the gaps regarding the scientific and clinical knowledge and how these can be used to outlook the future management of this disease will be also discussed.
