À medida que as pessoas envelhecem, as suas necessidades em matéria de saúde tendem a aumentar, devido sobretudo à complexidade das doenças crónicas. A institucionalização é, frequentemente a última opção e uma necessidade nos casos de maior dependência. Com a longevidade, ganha relevância o desenvolvimento de cuidados integrados, que atendam às necessidades de saúde e sociais destas pessoas. É neste contexto que as Estruturas Residenciais para Idosos – ERPI - têm procurado adaptar os seus serviços de forma holística, a estas novas realidades.
Compreender este processo é de suma importância quer do ponto de vista político, quer do interventivo, pois a elaboração de políticas de velhice adequadas (top-down) surge do conhecimento aprofundado da realidade (bottom-up). É com esta preocupação que este trabalho de projeto se desenvolve, considerando fundamental o atual papel das ERPI na prestação de cuidados específicos. Para tal, partiu-se da experiência profissional nesta área, como Assistente Social e Diretora Técnica numa ERPI, com o objetivo de realizar uma análise diagnóstica da situação vivida nessa ERPI e, posteriormente, uma proposta de melhoria da prestação dos cuidados de saúde e sociais. Em termos metodológicos foi efetuada uma recolha de dados dos processos individuais dos residentes e sistematizados num documento de Excel. Estes dados foram analisados e apresentados de forma descritiva para fundamentar o diagnóstico efetuado. Os dados recolhidos demonstram que a ERPI tem uma população muito envelhecida, com elevado grau de dependência e demências, necessitando de cuidados a vários níveis, prevenção, manutenção e reabilitação. Quanto mais dependentes se encontram as pessoas idosas, mais especializados e diferenciados são os cuidados recebidos, contudo ainda necessitam de ser melhorados, sobretudo no que diz respeito à individualização dos cuidados. Assim, propôs-se uma intervenção centrada na pessoa, num sistema integrado de prestação de cuidados, com vista a otimizar as oportunidades para o envelhecimento ativo na ERPI.
As people get older, their health needs tend to increase due to the complexity of chronic diseases. Institutionalization is often the last option, but for the highest dependence cases there are no other alternatives. The development of integrated care that meets the health and social needs of people becomes relevant as longevity increases. In this sense, the social facilities in long term (in Portuguese, “Estruturas Residenciais para Idosos” - ERPI) have been trying to improve their services in a holistic way, unlike in the past. Understanding this process is of crucial importance from the political and interventional point of view, since the elaboration of old-age policies (top-down) arises from a deep knowledge of reality (bottom- up). This work was developed based on the importance of the actual ERPI in specific health care. Thus, the experience of the author as Social Worker and Technical Director in an ERPI was used as a starting point to conduct a diagnostic analysis of the situation experienced in that ERPI and then elaborate a proposal to improve healthcare provision. In methodological terms, data were collected from individual resident processes and systematized in an Excel document. These data were analyzed and presented descriptively to support the diagnosis made. It was reached the conclusion that the ERPI has a very aged population, with high levels of dependency and dementia, requiring care at various levels, in terms of health prevention, maintenance and rehabilitation. The more dependent the older people are the more specialized and differentiated their care must be, however, they still need to be improved, especially as regards the individualization of care. A proposal for a person-centered care on integrated system was made, in order to optimize the possibility of active ageing in the ERPI.
