Escolhi o tema “Quality by Design” para a elaboração da minha tese, dado que é um termo amplamente usado em inúmeras áreas e com resultados muito positivos. Infelizmente, a sua associação com os sistemas de saúde não é muito pensada, sobretudo em Portugal, e mesmo quando são elaborados procedimentos e normas que permitem a melhoria contínua dos cuidados de saúde, estes muitas vezes não são seguidos, seja devido às pessoas ou às estruturas que não estão preparadas para suportar essas novas estratégias.
Sendo assim, decidi escolher um dos tópicos mais urgentes e mais falados dos cuidados de saúde, nos dias de hoje, devido ao perigo em que coloca a medicina do presente e futuro e os custos enormes que lhes estão associados, as Infeções Associadas aos Cuidados de Saúde (IACS) e as Resistências Associadas aos Microrganismos (RAM). Falo destes dois tópicos em conjunto, porque a solução do problema está na atuação sobre eles os dois concomitantemente, de modo a que se possa chegar ao objetivo pretendido. A redução de apenas um nunca chegará para resolver este problema mundial.
Esta tese tem o objetivo de, em primeiro lugar, fazer um ponto de situação em Portugal, sobre as IACS e a RAM, em segundo lugar de falar de quais as medidas que já foram adotadas em Portugal nestes últimos anos e propor algumas melhorias e por último, apresentar uma data de medidas inovadoras (seguidas internacionalmente ou não), cuja introdução nos hospitais portugueses pode ser de tremenda utilidade e fazerem parte da solução deste problema de saúde mundial.
I decided to choose the theme “Quality by Design” for my thesis, because it’s a term that is used in other areas of work, where it has shown great results, but is unfortunately often not associated to the health world — at least in Portugal. The work method, procedures and rules of the healthcare systems need to be rethought in order to get better results for quality of care and security for the patient. Thus, “Quality by Design” is a good way to reach those goals. With this in mind, I thought about the different areas in healthcare system that most needed this “redesign” and I came to the conclusion that the “Healthcare-associated infections” and the “Antimicrobial resistance” were the obvious choice, given the impact these problems have in terms of quality of care and, mainly, the security provided from the health systems and also the large fees associated nowadays, and in the future if the situation continues to get worse. This thesis is more focused on the situation in Portugal, but intends to deliver a system that can be used in any part of the world, because these issues are global threats. In the first place, this thesis makes reference to the state of the two problems (“Healthcare Acquired Infections” + “Antimicrobial Resistance”) in Portugal, showing the most recent data from “Direção Geral de Saúde” and some previsions made for the future, if this problem isn’t solved sooner or later. Furthermore, I go over the initiatives already taking place in Portugal and the procedures that are in use to combat the situation. Lastly, I offer some new solutions that may be of the most relevance to help fight this urgent situation in Portugal and even all over the world.
