Na literatura Ă© referido que pacientes com alexitimia tĂȘm resultados mais fracos em psicoterapia. A alexitimia Ă© caracterizada por: 1) dificuldade em identificar sentimentos; 2) dificuldade em comunicar sentimentos e; 3) pensamento orientado para o exterior. Neste trabalho propĂ”e-se que as caracterĂsticas associadas a este conceito reflectem deficits ao nĂvel dos processos de activação e de diferenciação emocional. Adicionalmente, sugere-se que com estes pacientes seja necessĂĄrio um maior enfoque terapĂȘutico ao nĂvel emocional e relacional. PropĂ”e-se dois estudos. Um estudo transversal, com o objectivo de verificar as diferenças ao nĂvel dos processos de activação e diferenciação emocional em pacientes alexitĂmicos. O segundo estudo, longitudinal, tem como objectivo verificar a associação entre alexitimia e qualidade da relação terapĂȘutica, bem como entre intervençÔes terapĂȘuticas relevantes e redução do grau de alexitimia. Pretende ser um trabalho nĂŁo orientado por um modelo terapĂȘutico especĂfico, mas por um metamodelo integrativo. Optou-se pela utilização do Modelo de Complementaridade ParadigmĂĄtica (M.C.P., Vasco, 2006). Para cada uma das hipĂłteses especula se sobre o significado de resultados, confirmatĂłrio e desconfirmatĂłrio. Discutem-se as implicaçÔes deste estudo no constructo da alexitimia e nas tomadas de decisĂŁo clĂnica. Sugere-se ter em conta esta dimensĂŁo na conceptualização de caso, no sentido de antecipar dificuldades ao nĂvel da relação e se optarem por objectivos mais focados nos processos subjacentes Ă alexitimia do que nas suas consequĂȘncias.
The literature points to the notion of patients with alexithymia as having poorer outcomes in psychotherapy. Alexithymia is characterized by: 1) difficulty in identifying feelings; 2) difficulty in expressing feelings and; 3) an external oriented cognitive style. In this project is hypothesised that alexithymia reflects deficits in the processes of emotional activation and differentiation. In addition, it's suggested that these patients need more emphasis on emotional and relational interventions. Two studies are discussed. One transversal study, with the aim of observing the differences in patients with and without alexithymia in emotional activation and differentiation. And a longitudinal study, with the aim of observing the association between alexithymia and the quality of the working alliance, as well as the impact of certain therapeutic interventions in the reduction of alexithymia. This study is not guided by a specific therapeutic orientation, but follows an integrative model. The integrative Meta-model of Paradigmatic Complementarity (M.C.P., Vasco, 2006) is the framework of this study. For each hypothesis is speculated the expected and nonexpected results. The implications of the study are discussed, both related to the alexitimia construct and regarding clinical decisions. It's suggested that this dimension should be taken into account in the case conceptualization to prevent poorer therapeutic alliances and to choose interventions focused on the processes rather than the consequences.
