A imunohemoterapia é uma especialidade médica que compreende a área da medicina transfusional que cresce rapidamente, modifica-se continuamente e que apresenta uma grande perspetiva de desenvolvimento futuro. O sangue é um bem escasso, que nem sempre está disponível nas quantidades pretendidas em alguns países. A transfusão de sangue está associada a riscos de etiologia diversa, como a transmissão de doenças infeciosas, as reações transfusionais, os efeitos potenciais de imunomodulação e aparecimento de agentes patogénicos emergentes. Mas até agora o recurso à transfusão sanguínea não é substituível, por isso o maior desafio da imunohemoterapia é possuir um sistema de qualidade efetivo com o foco na avaliação dos pontos críticos da cadeia transfusional.
A maximização da segurança de todas as etapas de transfusão sanguínea pode ser atingida com a implementação do sistema de hemovigilância integrado que permite a tomada de decisões à base de análise crítica de notificações recolhidas. Para que o sistema tenha sucesso, no entanto, é fundamental que os profissionais da saúde, ligados à transfusão sanguínea, participem ativamente no processo de notificação.
Apesar das dificuldades de implementação, o trabalho até agora realizado na área de hemovigilância nacional tem contribuído para melhoria da prestação de cuidados em medicina transfusional.
The immunohemotherapy is a medical specialty that comprises the area of transfusion medicine that grows rapidly, changes continuously and has a great perspective for future development. Blood is a scarce resource that is not always available in the required quantities in some countries. Blood transfusion is associated with risks of diverse etiology, as the transmission of infectious diseases, transfusion reactions, potential immunomodulation and appearance of emerging pathogens. But so far the use of blood transfusion is not replaceable, so the biggest challenge of immunohemotherapy is to have an effective quality system with the focus on the assessment of the critical points of the transfusion chain. Maximising the safety of all stages of blood transfusion can be achieved with the implementation of the integrated haemovigilance system that allows decision-based critical analysis of collected notifications. For the system to succeed, however, it is essential that health professionals linked to blood transfusion, actively participate in the reporting process. Despite implementation difficulties, the work done so far in the national haemovigilance area has contributed to improving the delivery of care in transfusion medicine.
