Incidentalomas da supra-renal são um espectro de entidades patológicas que tem em comum a presença de um nódulo com um diâmetro superior a 1cm na glândula supra-renal descoberta acidentalmente durante uma investigação imagiológica. Estima-se que a sua prevalência na população seja de 2%.
Com o aumento do uso de meios imagiológicos e a eficácia crescente destes, os clínicos vão se deparar cada vez mais com estas entidades. Dado a diversidade de etiologias responsáveis, com historias naturais radicalmente diferentes e a complexidade da investigação clinica, torna-se fundamental a adoção de uma medicina baseada na evidencia na abordagem aos incidentalomas da glândula supra-renal.
Este artigo de revisão visa assim agregar o conhecimento atual sobre incidentalomas abordando a sua prevalência, etiologias, historias naturais, avaliação, indicações cirúrgicas e de seguimento.
Adrenal incidentalomas are a spectrum of pathological entities that have in common the presence of a mass lesion greater than 1cm of diameter in the adrenal gland discovered accidentally during an unrelated imaging investigation. It’s prevalence in the population is about 2%. With the widespread use and efficiency of imaging technology, clinicians will increasingly face these entities. Given their diversity of etiologies, their radically different natural histories and complex clinical investigation, it becomes fundamental to take an evidence based medicine approach to the topic of adrenal incidentalomas. This review article aims to summarize the current state of understanding about adrenal incidentalomas focusing on its prevalence, etiologies, natural histories, evaluation, indications for surgery and follow up.
