Este projeto de investigação contempla um assunto central: uma seleção de “Ferramentas de otimização farmacoterapêutica em cuidados continuados integrados por intervenção farmacêutica.” Em termos de organização, a monografia é constituída por duas secções principais. Uma introdução teórica, que define e esclarece o campo de estudo e aborda o essencial para a compreensão do trabalho. São referidas a origem e definição de Cuidados Continuados Integrados, partindo de uma perspetiva geral e abrangente, e terminando na realidade portuguesa. É também referido o papel do farmacêutico e as suas intervenções nestas unidades. A segunda parte, o trabalho de investigação, contém um conjunto de vinte e quatro critérios e algoritmos selecionados através de uma revisão sistemática da literatura e uma pós-seleção de seis ferramentas, por três farmacêuticas a trabalhar em unidades da Rede Nacional de Cuidados Continuados Integrados (RNCCI). A seleção das seis ferramentas foi obtida através da Técnica de Grupo Nominal. Pretendeu-se, desta forma, alcançar um conjunto de ferramentas de otimização farmacoterapêutica, adaptadas e úteis à prática farmacêutica em unidades da RNCCI.
This research project addresses a core issue: a selection of “Pharmacotherapeutic optimization tools for Integrated Continuous Care useful to pharmaceutical intervention.” In terms of organization, monograph is divided into two main sections. First section is a theoretical introduction, on which the field of study is defined along with other important concepts for all work understanding. The origin and definition of Integrated Continuous Care is mentioned, starting from a general and comprehensive perspective to the Portuguese reality. The role of pharmacists and their real and potential interventions are also mentioned. The second part, the research project, contains a final set of twenty-four criteria and algorithms selected through a systematic literature review and a post-selection of six final tools by three pharmacists working in different units of the Portuguese Network for Integrated Continuous Care Long (RNCCI). Selection by professionals was assisted by the Nominal Group Technique (NGT), as a decision and consensus criteria. Thus, this monograph pursues to obtain a final set of pharmacotherapeutic optimization tools useful and adjusted to pharmacists work at RNCCI units.
