Introdução: a endometriose associa-se a eventos como inflamação sistémica, stress oxidativo e perfil lipídico aterogénico, condições que podem influenciar o desenvolvimento e progressão da doença coronária aterosclerótica. Esta revisão sistematizada pretende revisitar a informação sobre a associação entre endometriose e potencial aumento do risco coronário.
Material e métodos: recorreu-se à base de dados PubMed; foram utilizados como termos-chave: "endometriosis and cardiovascular risk”; “endometriosis and risk of heart disease”; “endometriosis [Mesh] and endothelium [Mesh] not "lymphatic vessels [Mesh]”.
Resultados: as mulheres com endometriose têm risco superior de enfarte agudo do miocárdio, angina, cirurgia de bypass coronário por enxerto, angioplastia coronária e colocação de stent, e apresentam valores inferiores de dilatação mediada pelo fluxo e valores superiores da velocidade de pulso, quando comparadas com os grupos controlo (mulheres sem endometriose).
Conclusões/Comentários: a endometriose está associada ao aumento do risco coronário, sobretudo em mulheres do grupo etário entre os 40 e os 55 anos. Esse aumento deve-se principalmente ao estado de inflamação crónica e à eventual intervenção médica, como a histerectomia e/ou ooforectomia, e ainda a potenciais fatores como: o recurso a anti-inflamatórios não esteroides e aos análogos das gonadotrofinas hipotalâmicas; e a suscetibilidade geneticamente determinada.
Introduction: endometriosis is associated with systemic inflammation, oxidative stress and atherogenic lipid profile, which have an important role in the development and progress of coronary atherosclerosis. This review intends to gather scientific evidence regarding the association between endometriosis and increased risk of coronary heart disease. Methods and materials: the research was made on the PubMed database where the following key-words were used: "endometriosis and cardiovascular risk”; “endometriosis and risk of heart disease”; “endometriosis [Mesh] and endothelium [Mesh] not "lymphatic vessels [Mesh]”. Results: women with endometriosis have an increased risk of acute myocardial infarction, angina pectoris, coronary artery bypass graft surgery, coronary angioplasty and stent placement. They have lower flow mediated dilatation and bigger pulse wave velocity than the control group. Conclusions/Comments: endometriosis is associated with an increased risk of coronary artery disease, mainly in women in their 40’s and 50’s years of age. This increase is mostly due to chronic inflammation, medical intervention (hysterectomy and/or oophorectomy) and other factors that may influence this association: non-steroidal inflammatory drugs, gonadotropin releasing hormone analogue drugs and genetically determined susceptibility.
