A evolução das ciĂȘncias mĂ©dicas permitiu um significativo desenvolvimento de tratamentos e procedimentos que conduziram a um aumento da esperança mĂ©dia de vida da população mundial. Desta forma, patologias como a sĂ©psis grave e o choque sĂ©ptico tornaram-se das principais causas de morte no mundo, estimando-se o aparecimento anual de 1,5 milhĂ”es de novos casos na AmĂ©rica do Norte e de 1,5 milhĂ”es na Europa. A sĂ©psis e o choque sĂ©ptico sĂŁo caracterizados por uma resposta exacerbada do sistema imunitĂĄrio, com libertação massiva de mediadores quĂmicos que estĂŁo na origem da apoptose celular, lesĂŁo do endotĂ©lio e falha multiorgĂąnica. Ainda nĂŁo existe uma forma concisa de diagnĂłstico, pelo que continua a ser baseado na experiĂȘncia dos profissionais de saĂșde e na avaliação dos sintomas apresentados, com a inclusĂŁo de factores clĂnicos, microbiolĂłgicos e biolĂłgicos. Para o tratamento, recomenda-se o uso de fluidos de reanimação e de fĂĄrmacos vasopressores e inotrĂłpicos, sempre que a primeira abordagem nĂŁo seja suficiente para estabilizar a hipotensĂŁo induzida pelo estado sĂ©ptico. Concluindo, futuramente novas abordagens serĂŁo necessĂĄrias de forma a colmatar os dĂ©fices existentes a nĂvel do diagnĂłstico e do tratamento, de forma a reduzir a mortalidade e morbilidade associada Ă sĂ©psis e choque sĂ©ptico.
The evolution of medical science has allowed a significant development of treatments and procedures that led to an increase in life expectancy of the world population. Thus, disorders such as severe sepsis and septic shock have become major causes of worldwide death, with an annual incidence of the 1.5 million new cases in North America and 1.5 million in Europe. Sepsis and septic shock are characterized by an extended response of the immune system with massive release of chemical mediators which cause cell apoptosis, endothelial injury and multiple organ failure. Although there is no concise diagnostic, this continues to be based on the experience of health professionals and the assessment of symptoms, with the inclusion of clinical, microbiological and biological factors. For treatment, it is recommended to use fluid resuscitation and vasopressors and inotropes when the first approach is not sufficient to stabilize the hypotension induced by sepsis. In conclusion, future new approaches will be needed in order to fill the existing gaps in terms of diagnosis and treatment to reduce mortality and morbidity associated with sepsis and septic shock.
