Hoje em dia sabe-se que a gravidez e o parto são dois momentos de grande stress metabólico para a mulher, pelo que podem contribuir para o agravamento clínico de pacientes com doença de Pompe e doença de Gaucher. Tendo em conta a inexistência de ensaios clínicos aleatorizados que avaliem a segurança do uso de terapêutica de substituição enzimática (TSE) durante a gravidez e a lactação, torna-se relevante analisar a literatura disponível até ao momento, nomeadamente múltiplos relatos de caso de mulheres grávidas sob TSE. Esta revisão aborda o impacto da TSE em mulheres grávidas com doença de Pompe e doença de Gaucher, tendo como objetivos: analisar o perfil de segurança na gravidez; avaliar a sua eficácia na estabilização da doença e, por fim, perceber o impacto da TSE nos diversos outcomes obstétricos e neonatais. Para o efeito, foram analisados 29 artigos científicos utilizando o PubMed.
Nowadays it is known that pregnancy and delivery are two moments of the woman’s life marked by an increase in the cellular’s metabolic stress, which can contribute for the clinical deterioration of Gaucher and Pompe’s disease patients. Taking this into account, so far there are no randomized clinical trials that evaluate the safety of enzyme replacement therapy (ERT) during pregnancy and lactation so, for this paper, it was important to analyze multiple case reports on the matter. This review addresses the impact of ERT on pregnant patients with Gaucher and Pompe’s disease and its ultimate goals are: analyze the safety profile of ERT as the role it plays on the disease’s stabilization and also the impact on obstetric and neonatal outcomes.
