Abstract(s)
O Transtorno do Espetro do Autismo é uma desordem do desenvolvimento que se caracteriza por uma tríade de défices na reciprocidade social, incapacidade de comunicação e padrões de comportamento restritos e repetitivos. Está descrito no DSM-V, onde se insere nas Desordens do Neurodesenvolvimento e engloba os diagnósticos de Síndrome do Autismo, Síndrome de Asperger e Desordem Difusa Não Especifica do Desenvolvimento. Surge tipicamente durante os primeiros três anos de vida, não tem cura, afeta mais o género masculino numa proporção de 4:1 e a sua etiologia permanece desconhecida embora fatores genéticos e ambientais sejam apontados como possíveis causas. O diagnóstico é feito com base em critérios específicos, testes e análise clínica. As crianças autistas apresentam inúmeros desafios relacionados com a consulta de Medicina Dentária, não só pela amplitude de comportamentos que apresentam mas também porque a consulta representa uma situação de stress para os mesmos. É comum apresentarem uma higiene oral deficiente, o que resulta num maior aparecimento de cáries, gengivites e periodontites. A criança autista tem ainda maior tendência para o bruxismo e para problemas ortodônticos. Existem inúmeros métodos de controlo de comportamento que podem ser aplicados e que têm vindo a ser desenvolvidos ao longo do tempo. Entre eles o Tell-Show-Do, reforço positivo e o TEACCH que recorre a visualização utilizando imagens em livros e cartões, vídeos e histórias sociais. O papel dos pais é de extrema importância pois são o elo de ligação entre a criança e o Médico Dentista, fornecem informação sobre os filhos que pode auxiliar no controlo de comportamento durante a consulta e são os responsáveis pela manutenção de uma boa saúde oral destas crianças quando motivados para tal. É essencial que os Médicos Dentistas estejam cientes do que este Transtorno representa e que as clínicas estejam adaptadas a estas crianças.
The Autism Spectrum Disorder is a developmental disorder characterized by a triad of deficits in social reciprocity, impairments in social skills and restrictive and repetitive behaviour standards. It is described in DSM-V under neurodevelopment disorders and includes the Autism Syndrome, Asperger Syndrome and Pervasive Neurodevelopment Disorder Not Otherwise Specified. It commonly appears during the first three years of life, it affects the male gender more than the feminine in a 4:1 proportion and it´s etymology is still unknown although genetic and environmental factors might be involved. The diagnosis is based on specific criteria, tests and clinical observation. Autistic children exhibit a number of challenges related to dental appointments, not only for the wide range of behaviour but also because the dental appointment represents a stressful situation for them. Parents role is highly important for they are the link between the child and the Dentist, giving information about their children that is crucial for behaviour control methods during dental procedures. They are also the main responsible for maintaining a good oral health care for their children, once they are motivated. It is imperative for Dentists to be aware of what this Syndrome represents and dental clinics must be adapted to receive these children.
