Os Cuidados Paliativos Pediátricos envolvem crianças e jovens portadoras de doença limitante/ameaçadora de vida, com necessidade de saúde complexas, independentemente da sua idade, diagnóstico, local de residência, cultura ou nível socioeconómico, focando-se na melhoria da qualidade de vida da criança/jovem e suporte à sua família.
É importante reconhecer que a esperança de vida de crianças com estas patologias tem vindo a aumentar e, portanto, quando se prevê sobrevivência até à idade adulta deve ser iniciada e preparada atempadamente a transição de cuidados, para que esta seja feita da forma mais profícua para os serviços de adultos.
Ao longo deste trabalho será feita uma breve revisão bibliográfica dos Cuidados Paliativos Pediátricos existentes atualmente, exploração dos desafios encontrados aquando da transição de cuidados para o serviço de adultos e análise de um caso clínico elucidativo desta realidade.
A transição de cuidados é um processo com alguns obstáculos ainda mais num país como Portugal, em que até há pouco tempo não tinha provisão oficial de Cuidados Paliativos Pediátricos, e em que surge cada vez mais esta necessidade de transição de cuidados. Perante isto, propõem-se uma revisão da literatura existente acerca da transição de Cuidados Paliativos Pediátricos para Cuidados Paliativos de adultos.
Pediatric Palliative Care involves children and young people with a life-limiting/life-threatening illness, with complex health needs, regardless of their age, diagnosis, place of residence, culture or socioeconomic level, focusing on improving the child's quality of life /youth and support your family. It is important to recognize that the life expectancy of children with these pathologies has been increasing and, therefore, when survival into adulthood is expected, the transition of care must be initiated and prepared in a timely manner, so that it is done in the most fruitful way for adult services. Throughout this work, a brief bibliographical review of currently existing Pediatric Palliative Care will be carried out, an exploration of the challenges encountered when transitioning from care to an adult service and an analysis of a clinical case that illustrates this reality. Care transition is a process with some obstacles, even more so in a country like Portugal, where until recently there was no official provision of Pediatric Palliative Care, and where this need for care transition is increasingly emerging. In view of this, we propose a review of the existing literature on the transition from Pediatric Palliative Care to Palliative Care for adults.
